Mandatory Health Insurance

Dear student:

Central State University (CSU) is concerned about your health and safety and the health and safety of your classmates. To assist students, the University has developed the Central State University Student Health Plan. The Plan provides coverage for sickness and injuries that occur on and off campus, and include special cost-saving features to keep the coverage as affordable as possible. The Plan also offers a 24 hour Nurse Hotline for your convenience: 800-557-0309.

As a full-time student enrolled at CSU, you are automatically enrolled in the Plan. A student is considered full-time if they are taking 12 or more credit hours. The Plan is also available to part-students for an additional fee. This can be accomplished through the CSU Bursars Office.

The Plan provider is Consolidated Health Plans (CHP). Plan coverage for the 2013-2014 Academic year begins August 11, 2013. Pamphlets with Plan details are available at the Student Health Center. Plan details are also available through the website:

Please remember you must use the Student Health Center first for all medical problems unless you have additional health insurance which will pay your bills.

As always, we wish you safe and healthy times and look forward to seeing you on campus. Please contact the Student Health Center staff at (937) 376-6134 if you have any questions or concerns.
